Collaborative Forms

Collaborative Forms

SimplyDoc telehealth appointments can include custom collaborative forms. In this video, Arin Sime, Founder and CEO of SimplyDoc, shows how these forms work and can be customized to your needs.

Forms are accessible on the doctor’s side of the telehealth visit. All the doctor needs to do is select the appropriate form to open it for both users and begin completing it together.

Forms appear on the screens of both the patient and the provider. Similar to Google Docs, each individual can see the form fields update as they are being completed by the other person.

Forms can be customized to your needs. Whether you need an informed consent form, an intake form, a medical history form, or something else, we can create the forms just the way you need them.

Forms can include digital patient signatures. This is an easy addition to any form. The patient’s signature will appear on both screens, and it will be saved with the form and included in a download.

Forms can be easily accessed by the doctor after the call. They are stored in the patient’s profile and can be downloaded later.
